Tanyeli1 . 1 Cukurova University, Turkey To describe the clinical course of pediatric neutropenic oncology patients undergoing single dose granulocyte transfusion (GTF). Retrospective chart review including all children receiving granulocyte transfusion between September 2009 and September 2011 in a University Hospital setting. Thirty seven patients received a total of 51 courses of GTFs. WBC count was assessed before and 2 h and 24 h post transfusion. In thirty five courses of GTFs patients had leukemia and in sixteen courses of GTFs patients had solid tumors. 80.4% (n = 41) of the patients were not in remission. Mean duration of fever and neutropenia was 18±17 days. The indications for granulocyte transfusions were bacterial infection (43%, n = 22), fever of unknown origin (41.2%, n = 21), fungal infection (9.8%, n = 5), both bacterial and fungal infection (2%, n = 1), antibiotic resistant bacteremia (2%, n = 1) and prophylactic (2%, n = 1). Granulocyte yield was 3.1±1.2×1010 , and mean WBC count before, 2 h and 24 h after transfusion were 465±522 mm3 , 4411±6790 mm3 , 3525±4022mm3 respectively. GTFs were well tolerated except one patient who had fever during transfusion. Thirty nine (76.5%) of the courses resolved from infection and discharged from the hospital. Of the 12 courses (23.5%) who died, seven of them were refractory to treatment, two of them were new diagnosis and three of them had relapsed disease. This case series documents the course of 37 septic neutropenic pediatric oncology patients who underwent a total of 51 GTF courses. GTFs were generally well tolerated and improve short term outcome in neutropenic pediatric oncology patients.
