The nursing home residents demonstrated significantly lower neuropsychological testing across multiple domains as compared with a reference population of community-dwelling counterparts of the same age. Conclusions: People living in nursing homes were found to be highly medical and psychological impaired especially with low test scores in quality of live. The selected tests were not able to find differences between diabetics and non diabetics. This may contribute to the low sensitivity of the test battery in elderly people. On the other hand diabetes in the elderly may be of minor importance for impairment, complication rate and quality of life especially in nursing homes.

Metformin has been found to improve ovulation rates and to reduce hyperandrogenism in women with PCOS, but it has been argued that this occur largely because of weight loss. However, weight loss does not occur in all women and the mechanisms for the ovulation promoting effect of metformin remains unclear. To this end, we studied 30 anovulatory women with PCOS and treated them either with placebo (n=lO), metformin 1500 mg/day (n=lO) or rosiglitazone 8 mg/day (n= 10). Changes in insulin resistance (IR), body mass index (BMI) and hormonal levels were evaluated in concert with evidence of ovulation (as confirmed by finding progesterone levels >7 rig/ml in the presumed luteal phase). All 3 groups had similar BMI, androgen levels and IR. Fasting blood was obtained for LH, FSH, estradiol, testosterone, insulin, glucose and liver enzymes at baseline and for 3 months. Measurements were carried out on days 5-6 after menses and serum progesterone was measured on cycle days 21-23 after spontaneous menses. IR was calculated by quantitative insulin sensitivity check index method (QUICKI). After 3 months BMI, LH and FSH remained unchanged in the 3 groups. as did E2. Testosterone levels decreased with metformin (83210 to 76215 ng/dl) and rosiglitazone (85flO to 75f20 ng/dl), but not significantly. High prevalence of vitamin D deficiency has been reported in many countries independently of sunlight exposure. In this study we assessed the prevalence of vitamin D deficiency between Sicilian postmenopausal women. Two hundred consecutive unselected postmenopausal women (aged 48-65 years, mean age 56.720.4 yrs), coming for a screening evaluation to a menopausal center, were studied. The 25-hydroxy vitaminD (25.OH-vitD) serum levels were measured and compared with those of a selected (nurses and doctors) group of 45 postmenopausal women who had normal bone mass (T score between +0.5 and -0.9) and similar age and body weight (control subjects). Mean serum levels of 25.OH-vitD were similar in the 2 groups of women. The 12.8% of unselected postmenopausal women had an absolute deficiency of 25-OHvitamin D (values lower than -2SD of control women, <17.8 rig/ml), while 17% had a relative deficiency of vitamin D (values between mean -lSD and mean -2SD of control women, c22.6 nglml and >17.8 rig/ml). A relative deficiency of 25-OH-vitamin D was also found in 2 control women (4.4%). A 60-year-old man with advanced liver cirrhosis due to hepatitis C and idiopathic dilated cardiomyopathy came to our department with hyponatremia 112 meq/L, low blood pressure and general weakness, Two months before current hospitalization the patient admitted to our institution with bleeding esophageal varices leading to shock, hepatic encephalopathy and requiring repeated sclerotherapy. In few hours after admission he developed fever and chills and initial diagnosis of septic shock was made. His hyponatremia was attributed to liver cirrhosis. The patient was treated with broad spectrum antibiotics, fluids and dopamine infusion. There were no signs of infection and urine sodium level was unusually high for cirrhotic patient. After 48 hours of treatment the patient still required a large amount of saline to maintain normal blood pressure. Unexplained hyponatremia along with profound hypovolemia and lack of evidence of infection suggested possible adrenal insufficiency.
