Many have chosen not to provide services at reimbursement tenns as low as the average wholesale price less 15%, plus a $1.25 dispensing fee, but in those states phannacists are guaranteed access to the program nonetheless. To ease communication with patients who may be deciding to change phannacies because of the information they have received from Blue Cross/Blue Shield or PCS Inc., the plan administrators, APhA suggests that phannacists post a sign in their phannacies stating: "The new Federal Employee Health Plan, which became effective January 1, 1993, includes important changes in your benefit for prescription medications. Although the plan designates phannacies as 'Preferred Phannacy Providers,' you are not required to use these phannacies. However, your reimbursement may be reduced for prescriptions dispensed by nonpreferred providers. Speak to your phannacist for details." APhA will continue to study this program and the issue of payment systems in phannacy. The Association is committed to working on reimbursement plans that will facilitate phannacists' delivery of quality phannaceutical care services.

Affiliation is an arrangement between state associations and APhA to work more closely to further their common interests and the interests of the profession. APhA's State affiliates also have a stronger voice in the Association's House of Delegates and policy-making process. In her letter announcing the affiliation, NMPA President Tris McSherry wrote, "We feel that NMPA and APhA are generally in agreement on the major issues affecting the practice of phannacy.... The ability of the profession to work together will determine the success of our efforts." McSherry thanked APhA for its support and said she and her association "look forward to an even better relationship in the future."

APhA is working with the American Society of Hospital Phannacists (ASHP) to draft a set of guidelines for pharmacists who compound phannaceuticals in daily practice. The "Draft Guidelines on Compounding of Nonsterile Products in Phannacies" will list practice standards allowing phannacists to assess whether or not their practices are equipped to handle extemporaneous compounding. The guidelines address such factors as equipment and facilities, ingredient selection, personnel, recordkeeping, stability and expiration dating, and packaging and labeling. An open hearing to discuss the draft guidelines was held at APhA's Annual Meeting last month in Dallas, Tex. A second will be held at ASHP's Annual Meeting in June 1993. APhA and ASHP expect the guidelines to be fmal by the end of the year.

Also a past executive director of the Arizona Phannacy Associaton, Boesen had been assistant vice president of professional services at PCS. Daniel E. area of OTCs. Their input, it seems to me, would be extremely valuable to the committee's deliberations. For example, practicing pharmacists recently identified that women coming in for the new OTC vaginal antifungal creams could actually be showing symptoms of a much more serious underlying disorder such as HIV infection. Other practicing pharmacists have identified the problem associated with laxatives, or ipecac, and eating disorders. The new Omnibus Budget Reconciliation Act of 1990, drug use review, and counseling requirements require practicing pharmacists to make a reasonable effort to predict and prevent drug-drug interactions, including interactions with OTC products. If federal law requires pharmacists to consider their patients' OTC drug use, then federal agencies should make every effort to seek advice from practicing pharmacists in this area. jeff M. jellin, PharmD Editor, Pharmacist's Letter

Legislation Needed for Chains to Change The February 1993 article "New Services Emerge in Chain Pharmacy" (p. 23) reviewed patient-oriented services that chain pharmacies now offer to comply

with the Omnibus Budget Reconciliation Act of 1990 (OBRA '90). This article underscores the fact that it takes the extraordinary means of legislation to force the chain drugstores to allow us a professional practice. I believe that a pharmacist is a health care professional. I do not believe, however, that chain pharmacies and the National Association of Chain Drug Stores (NACDS) share this belief. Many interns and one district manager have told me that many chains actively discourage pharmacists from doing anything other than dispense medications because only dispensing brings in money. Recently, the California Pharmacists Association discussed with members the need for collective bargaining, because many of its chain pharmacist members are being denied reasonable working conditions. In Arizona, the chain drugstore industry attempted to influence appointees to the Board of Pharmacy in an effort to keep pharmacists at the counting tray. 1 NACDS would prefer us to be much less educated than most of us are comfortable with. A reasonable balance is needed between practicing professionally, producing revenues, and caring for the patient. OBRA '90 is forcing the chains to stop directing us according to their profit margin and let us practice
