L E T T E R TO T H E E D I T O R Dear Sir, In the issue of " T h e H a n d " for February, 1973 (Vol. 5 No. 1, p. 52), Nathan and Schlein record a case of multiple dislocations on a single finger. They comment upon the rarity of this condition and its absence from the English literature. In the interests of accuracy I write to draw your attention to the article which I wrote for this journal on Injuries of the Metacarpo-phalangeal Joints in September, 1971 (Vol. 3, No. 2, p. 159). This includes an illustration of a double dislocation of the interphalangeal joint on a single digit and it may be of interest to record the patient's explanation of how it occurred. She stated that there was only one dislocation until her doctor attempted to reduce it, thereby causing a second. The patient was an Irish lady of witty disposition and on friendly terms with her doctor. I do not know whether her explanation is correct but she took the episode in good part and needless to say made a rapid recovery once the dislocations were reduced. It had not occurred to me that this was a very rare situation and I doubt whether it is so.

There have been many books recently on the subject of internal fixation of the skeleton of the hand, using the Swiss A.O. system. This monograph gives the author's wide experience in this method, and I consider it shows a better balance of judgement in using a wider range of methods than his predecessors. It is my experience that open operation is very seldom required for fixation of fractures in the hand, but the methods necessary are here and as the plates and screws become smaller, their use will become more justifiable.
