She was treated with chemotherapy last year because systemic affectation; the patient has complained about intense pain and swelling of gums beginning 2 years ago. Intraoral examination reveals poor oral hygiene, gingival enlargement, and multiple necrotic ulcerations covered by pseudomembrane involving the upper buccal gingiva, the intercanine region, and the lower buccal and lingual gingiva of premolars, canines, and incisives. The orthopantomography reveals generalized alveolar bone loss. A biopsy was performed. The presence of a mixed inflammatory infiltration, and immunohistochemical staining positive for langerin and CD1 a confirms the diagnosis of the histiocytosis X. We proceed to the intralesional infiltration with triamcinolone acetonide 40 mg/mL once weekly for 4 weeks, and the complete cure of the oral lesions was achieved. Six months later no lesions were observed.

Anamnesis revealed smoking and chronic alcoholism for 60 years. At the extraoral examination, submandibular lymph nodes were bilaterally enlarged, hardened, and fixed to palpation. Intraoral examination revealed total edentulism and extensive ulcer on the anterior mouth floor, involving the ventral surface of the tongue and anterior inferior alveolar ridge with volumetric enlargement, tongue without mobility, hardened to palpation, pain, and limitation of oral opening. Panoramic radiograph showed a radiolucent image in the anterior region of the mandible with irregular bone destruction similar to "moth-eaten" and approximately 4 cm in the largest diameter. Diagnostic hypothesis was squamous cell carcinoma. Incisional biopsy was performed. The histopathologic report confirmed the diagnosis of basaloid pattern squamous cells carcinoma. The patient was referred for cancer treatment.

ORAL MANIFESTATION OF PRIMARY SYPHILIS LASTING 4 MONTHS. LETICIA e DRUMOND DE ABREU GUIMARAES, MATHEUS SAMPAIO DE OLIVEIRA, THALITA SANTANA e CONCEIC ¸ AO and, SUZANA CANTANHEDE ORSINI MACHADO DE SOUSA Syphilis, a sexually transmitted infection, has been increasing in incidence over the years, especially among the young adult population. The oral manifestation of primary syphilis is usually characterized by short evolution. In this report we present a case of an oral manifestation of a primary syphilitic lesion lasting 4 months, and we emphasize the importance of the correct diagnosis. A 22-year-old woman presented for evaluation of a growing single painful oral lesion with an evolution of 4 months. At clinical examination an extensive ulceration was observed, covered by a fibrinous membrane, located on the lateral border of the tongue extending from the apex to the base and ventral surfaces.

OOOO January 2020 A biopsy was performed, and serologic tests were requested. The results showed positivity for Venereal Disease Research Laboratory and fluorescent treponemal antibody absorption and antitreponema in the biopsied material, confirming the diagnosis of syphilis. Antibiotic therapy was started with total remission of the lesion.

EXTENSIVE MALIGNANT EPITHELIAL LESION IN PATIENT WITH CHRONIC RENAL DISEASE. Anamnesis revealed stage IV chronic renal disease, myocardial revascularization surgery, gastritis, depression, allergic rhinitis, recent weight loss, and previous smoking stopped 20 years ago. The extraoral examination showed facial asymmetry due to a swelling in the right mandibular body covered by normal skin. Intraoral examination evidenced an exophytic ulcer on the right edentulous mandibular alveolar ridge, extending from the parasymphysis to inferior retromolar region with verrucous aspect and irregular borders, hardened and painful to the palpation, and 7 £ 3 cm in its largest diameter. Smear cytolopathology and incisional biopsy were performed. Diagnostic hypothesis was squamous cell carcinoma. The cytopathologic study revealed atypical cells strongly suggestive of malignant neoplasm. Histopathologic analysis confirmed the final diagnosis of moderately differentiated squamous cell carcinoma. The patient was referred for cancer treatment.

RESPONSE AFTER ANTIRETROVIRALS AND CHEMOTHERAPY IN HIV-POSITIVE PATIENT WITH MULTIPLE KAPOSI SARCOMAS OF ORAL CAVITY. Anamnesis showed systemic arterial hypertension. Extraoral examination revealed enlarged right submandibular lymph nodes. Intraoral examination revealed red-purple ulcerated swellings in the anterior right vestibular and lingual gingiva, vestibular gingiva of teeth 14, 13, 11, and 21, and bilateral hard palate with ulcer on the right side; the right soft palate and tuberosity were also committed. Diagnostic hypothesis was Kaposi sarcoma associated with HIV infection. Incisional biopsy was performed. Histopathologic analysis showed hypercellular fasciculated fibrous connective tissue in submucosa, exhibiting altered blood vessels appearance and vascular proliferation with chronic inflammatory infiltrate and extravascular erythrocytes.
