Question: For approximately 4 years, a 70year-old woman had chronic intermittent epigastric pain that became worse with eating and occasionally awoke her at night. Physical examination and laboratory data, including serum amylase, lipase, and liver function tests, were unremarkable. Chest radiograph revealed a left pleural effusion and computed tomography of the abdomen showed a slightly edematous pancreatic tail with an adjacent fluid collection. Endoscopic retrograde cholangiopancreatography with cannulation

and contrast injection of the minor papilla was performed (shown in the figure). What is the diagnosis?
