“Parathyroid surgery can be safely performed in a community hospital by experienced parathyroid surgeons: A retrospective cohort study”
Dear Sir, We read with interest the article by Vladimir K. Neychev et al. “Parathyroid surgery can be safely performed in a community hospital by experienced parathyroid surgeons: A retrospective cohort study” [1]. We congratulate the authors on their magnificent effort to perform parathyroid surgery in the community health centre and there by taking endocrine surgery to areas under accessed. This kind of initiative where trained surgeons under the mentorship of expert endocrine surgeons perform peripheral surgeries with acceptable results shall result in presence of quality health care givers even in the periphery. We would like to make certain comments. We agree with the authors that surgeon is one of the important factors for successful parathyroidectomy. But even the radiologist and nuclear medicine physician if they are dedicated to endocrine surgery then the imaging interpretation is much better. Parathyroid disorders even though common, requires high index of suspicion on part of the physician examining the patient, so that the diagnosis can be made at an initial stage itself and there by complications and squeal can be prevented. In this era of tele mentoring and tele consultation all specialties including para clinical and clinical specialties should make use of tele medicine facilities in complex cases for betterment of treatment and patient care. We have few queries which can be of interest to future readers. Did the authors perform intra op frozen section during surgical procedures? Did the endocrine surgeons perform surgeon performed ultrasound [2] and did the findings correlated with the radiologist.
Did the authors perform any of their surgeries under local anesthesia? [3].
