EFFICACY OF TOTAL PARENTERAL NUTRITION (TPN) AS AN ADJUNCT TO HEAVY CHEMOTHERAPY- A RANDOMIZED PROSPECTIVE TRIAL. C. Drott, B. Unsgaard & K. Lundholm, Departments of Surgery & Oncology, Sahlgrenska Hospital, Gothenburg, Sweden. The role of nutritional support as an adjunct treatment to chemotherapy is unclear. Aim: To evaluate whether chemotherapy induces toxic blockade on protein metabolism and to assess the effects of adjuvant TPN on body composition in well-nourished patients with a curable malignant disease. Methods: 23 patients with non-seminomatous germ cell tumors of the testis were randomized to adjuvant TPN or spontaneous food intake during a 10 week course of CisplatinumVinblastine-Bleomycin (CVB) treatment. Patients randomized to adjuvant nutrition received TPN during every third week in conjunction48 ith admission for chemotherapy administK whole body counter, antropometry, toration. Food intake, body composition (TBK by tal body water), nitrogen balance, energy expenditure and 3-methylhistidine excretion were measured. Results: The loss of body weight and TBK after treatment and the hematologic side effects Urinary excretion of 3-methylhistiduring treatment were not different between groups. dine was reduced by 22% despite TPN indicating a-reduction-of protein turnover. Nitrogen balance was negative in the spontaneous intake group but positive in the TPN group. increasing the protein content of TPN resulted in a further improved nitrogen balance. Spontaneous food intake was severely reduced. This effect was also evident in both groups between chemotherapy administration due to the prolonged anorectic effect of chemotherapy. Conclusion: Chemotherapy does not prevent resynthesis of body proteins. However, despitl the positive nitrogen balance during TPN administration, adjuvant TPN did not translate into protection of body composition. This was due to the anorexia at home between treatments. Short term TPN is without obvious benefit as adjunct to chemotherapy.
