Lapin and Fred: Postpartum Pneumoperitoneum, p. 328. A patient who sought medical care 6 weeks post partum because of pleuritic pain is presented. Chest film revealed pneumoperitoneum and exploratory laparotomy was performed with the presumptive diagnosis of ruptured gas cyst of the intestines. There were no demonstrative abnormalities of the viscera at operation. The incidence of postpartum pneumoperitoneum (PPP) is not known. The presenting symptom is usually pleuritic pain over the lower portion of the chest anteriorly, and over the upper abdomen. Pain in the shoulders may also occur, and develops shortly after knee-chest exercises or similar movements. PPP may occur as early as the ninth and as late as the thirty-sixth postpartum day. The exact mechanism which produces pneumoperitoneum is not clear, and the authors postulate that when a woman arises from the knee-chest position, the labia close, the vaginai waiis come together, and air may be forced into the abdominal cavity through the Fallopian tubes of the patulous postpartum genital tract. In the postpartum woman, with radiographic evidence of pneumoperitoneum, the presence of a relative sense of well-being, and a history of knee-chest exercises or related exercises, the diagnosis of postpartum pneumoperitoneum should be seriously considered. ¥-lith cor1.ser:.1ative mar...agement, i.e., bed rest and supportive care, symptoms will usually subside in 24 to 72 hours.
