41-year-old patient whose only medical history entry was superficial thrombophlebitis in the right great saphenous vein. Patient consulted our service for flaccid, wrinkled and atrophic skin with well-defined edges, of 7 cm in diameter and a 2-year history of progressive growth (Fig. 1). A skin biopsy was performed, observing an almost total absence of elastic fibres with Van Gieson staining, compatible with anetoderma (Fig. 2). Additional tests carried out only showed a protein S deficiency, which was confirmed in a second measurement. The primary anetoderma is a rare alteration of elastic fibres closely associated with the antiphospholipid syndrome and lupus erythematosus. There have been isolated reports of association with other prothrombotic states, such as protein C and S deficiency, hyperhomocysteinemia and antithrombin iii deficiency. Therefore, a complete thrombophilia study and a lupus erythematosus screening would be appropriate after the diagnosis of primary anetoderma.
