A 73-year-old woman presented with abdominal distension, fever, confusion, and acute renal failure. She had diabetes with a baseline serum creatinine level of 0.7 mg/dL (62 mol/L). Examination revealed a pelvic mass up to the level of the umbilicus. One day later, her serum creatinine was 3.1 mg/dL (274 mol/L). An ultrasound of the pelvis (Fig 54A) and an abdominal x-ray (Fig 54B) were obtained.
