This procedure can be done laparoscopically to reduce the size of the incisiondusually for an end-to-end reconstruction for a short stricture at the pelvic brim. Furthermore, when combined with aggressive Boari bladder flap mobilization, virtually the entire ureter can be repaired without using bowel segments. Allen F. Morey, MD

As expected, variability and overuse are common. A major question is whether continuation of antibiotic prophylaxis is warranted from time of surgery through catheter removal, which is a common practice and is outside the American Urological Association recommendations. Another issue is whether antibiotics should instead be given only at catheter removal, which appears to be warranted based on available published data from prostatectomy series. As the title indicates, opportunities for stewardship and standardization abound regarding antibiotic prophylaxis issues in reconstructive urology. Allen F. Morey, MD
