Nonetheless, this novel end point seems to prove effective in measuring erectile function in this study and falls in line with the standard end points. Abstract available at http://jurology.com/ Editorial Comment: This 12-week, double-blind, placebo controlled study investigated once daily tadalafil (2.5 mg titrated to 5 mg, or 5 mg) in 176 patients and 79 controls regarding confidence and perceived difficulty in performing sexual intercourse. Subjects were men with erectile dysfunction who were incomplete responders to as needed phosphodiesterase type 5 inhibitor therapy. The CPSIQ (Confidence in Performing Sexual Intercourse Questionnaire) and DPSIQ (Difficulty in Performing Sexual Intercourse Questionnaire) were administered at baseline and at 12 weeks. Mean change in CPSIQ for the tadalafil group was 1.8, which represents a shift from “very low” to “moderate” sexual confidence vs a mean change of 0.5 in the placebo group (p <0.0001). Mean change in DPSIQ for tadalafil was 1.6, which represents a shift from “very difficult” to “moderately” or “slightly difficult” sexual performance vs a mean change of 0.4 in the placebo group (p <0.0001). Once daily tadalafil vs placebo improves confidence and decreases difficulty in performing sexual intercourse for men with erectile dysfunction who were incomplete responders to as needed phosphodiesterase type 5 inhibitor therapy. The CPSIQ and DPSIQ are novel measures of confidence and difficulty in performing sexual intercourse. Both questionnaires would seem to have a role in understanding male erectile dysfunction. Abstract available at http://jurology.com/ Editorial Comment: These are interesting data and highlight an under studied area, that of “asexuality.” Approximately 37% of the 1,581 Chinese men surveyed acknowledged that they did not have sexual intercourse for at least 6 months. The specific reasons given included “erectile difficulty” (52.9%), “do not care about sexuality” (53.5%), “no longer necessary to have sexuality at this age” (47.7%), “severe stress” (44.4%), “severe fatigue” (26.3%) and “masturbation” (26.9%). There was no mention of partner status, which may have had an important role. Further study of this phenomenon is warranted. Allen D. Seftel, MD

Abstract available at http://jurology.com/ Editorial Comment: These authors provide insightful suggestions for ancillary maneuvers, called “straightening maneuvers,” during selected cases of penile prosthesis insertion. Within the straightening maneuvers group modeling was performed in 40 cases (43%), tunical relaxing incisions in 37 (39.8%) and tunical reconstruction in 16 (17.2%), most commonly using allograft dermis or pericardium, or synthetic polytetrafluoroethylene grafts. There were no significant differences in terms of device infection (p ¼ 0.15), mechanical failure (p ¼ 0.23) or erosion (p ¼ 0.96). This study suggests that in men with penile deformity requiring complex reconstruction to achieve straightening inflatable penile prosthesis implantation may be done proficiently and without increased adverse outcome risk. They report on 89 patients who underwent corporoplasty/straightening surgery using penile plication for Peyronie disease between 1990 and 2012. Followup data included 1) correction of the curvature, 2) any penile shortening, 3) sexual function and 4) complications. Mean followup was 103 months. Complete correction of the curvature was achieved in 81 patients (91%). Shortening of the penis (1.5 to less than 3 cm) was observed in 20 patients (22.5%), and 79 (88.7%) had good erectile function (International Index of Erectile Function score greater than 5 to 21). The most frequent complications were decreased sensitivity of the glans (8 patients, 8.9%), which resolved within about a year postoperatively (mean 11 months), and penile shortening (20 patients, 22.5%), which did not result in problems during sexual intercourse. These data provide reassurance that the corporeal plication procedure for Peyronie disease remains a viable surgical option. Allen D. Seftel, MD
