The growth hormone (GH)-insulin-like growth factor I (IGF-I) axis and leptin both play an important role in the regulation of body composition. The physiological interactions between leptin and the GH-IGF-I axis remain unknown. In patients with chronic renal failure, levels of both leptin and GH tend to be increased. The aim of this study was to investigate the relationships between circulating leptin and key elements of the GH-IGF-I system in patients with chronic renal failure. A total of 205 patients with chronic renal failure (79 receiving conservative therapy, 75 peritoneal dialysis and 51 haemodialysis) were studied. Levels of leptin, insulin, GH, IGF-I and IGF-binding protein-3 were measured using radioimmunoassay, in the basal state after an overnight fast. Statistical Package for the Social Sciences (SPSS) was used for statistical analysis. Univariate analysis and multiple

linear regression analysis were performed. Leptin levels were higher in patients treated with peritoneal dialysis (median, 36 ~g/1) than in those undergoing conservative therapy (10.8 pg/1) or haemodialysis (5.4 ~g/1; P < 0.0005). IGF-I levels were higher in patients undergoing peritoneal dialysis (median, 337 txg/1)than in those undergoing conservative therapy (247 vg/1) or haemodialysis (211 ~g/1; P < 0.001). Leptin and IGF-I levels were significantly correlated (univariate) in patients undergoing peritoneal dialysis (r= 0.3, P < 0.01). For the entire study group, IGF-I levels were significantly correlated (multivariate) with serum leptin levels, after controlling for gender and body mass index. In conclusion, leptin and IGF-I levels are significantly correlated in patients with chronic renal failure, suggesting an important physiological link between the GH-IGF-I axis and leptin.
