Narcolepsy is a sleep d~sorder characterized by excessive daytime sleepiness and abnor~litie~ Of rapid aye m o v ~ m t . Cataplexy and other ancillary symptoms may also be present. Strong association between D R 2 / D ~ I and narcolepsy emon~ Japanese and some Caucasian ~opulations, up t O 100%, ha~ been reported. Serological typing and HLA class XX s e ~ e n c e specific oligonucleotide were used t o study t~e genetic etiology oE narcolepsy in blacks and w h i t e . Patients ~ r ~ stratified based on family h i s t o r y , r a ~ , and cataplexy + / - . Of 71 subjects studied, 2 patients with cataple~y and 23 patients without oataplexy r e p o r t ~ an affected first d ~ r e e relative. 13 white and 8 black narcoleptics without cataple~y were DR2 ( - ) . 8 whites and 1 black patient with cataple~y ~era DQwI(-). 1 black and 3 white patients with cataplexy and 5 white patients without oataple~- were DR2 (-} and D~wl(-} by both serology and DNA testing. ~n narcoleptics were compared to controls no significant difference in the frequency Of DR2(+) and Dc~l(+} was f ~ (odds r a t i o 1. 082, 0.788 ; 1.531, 1.790; with & ~ithout c a t a p l ~ y respectively Eor DR2 end D q ~ . Xdentificatlon o f narcoleptics who were both DR2 and DQwl negative, both ~ith and W i t h o u t cataplexy, using two different typing methodologies suggests that DR2 is not the actual ~ene predisposing to narcolepsy in either blacks or whites.
