Beheshti Hospital Yasuj University of Medical Sciences, Dept. of Urology and Renal Transplantation, Yasuj, Iran INTRODUCTION & OBJECTIVES: Since its introduction in 1976, PCNL has had evolution both in techniques and equipments. post-operative nephrostomy and tubeless PCNL are both in use currently. The aim of this study was to evaluate the feasibility of Tubeless stenting PCNL. MATERIAL & METHODS: Since January 2010 till January 2013, 48 adult patients including 30 males and 18 females were enrolled in this study. All had incomplete staghorn stones. complete staghorn stones were excluded. PCNL position was prone. A 5 F ureteral catheter was inserted in all patients for contrast instillation and after finishing the procedure this catheter was used as a guide to insert an ategrade DJ stenT that replaced the ureteral catheter. The DJ catheter will be removed whithin one to 2 weeks after operation. a urinary catheter drains the bladder for approximately one day after the surgery. Other parts of the procedure was as routine. RESULTS: In 4 patients it was impossible to insert a DJ catheter and the post operative period was uneventful in all. In 2 patients a 20F catheter was inserted as a nephrostomy tube(conversion to standard procedure) due to possibility of significant bleeding. In the other 42 patients tubeless stenting PCNL was performed successfully. Some patients had post operative irritative urinary symptoms, otherwise no significant complication was encountered. CONCLUSIONS: The results of this study showed that in selected cases especially in larg single stones and smaller stones resistant to shock wave lithotripsy, PCNL can be done as a tubeless stenting procedure.
