Background: Sigmoid volvulus is a well-known and rather rare and potentially even life-threatening condition in children. Treatment consists of a vigorous rectal enema, eventually followed by surgery if the former management is not successful. The goal of this presentation is to demonstrate exclusive pictures delivering unequivocal direct evidence for both volvulation and successful devolvulation of a classical sigmoid volvulus. Despite a good search we have not found a similarly powerful e because self-explanatory e illustration of a phenomenon that is far better known than actually documented. Ó 2013 The Authors. We report on a one-year-old boy presenting with acute abdominal pain and non-bilious vomiting. Plain radiograph after insertion of a rectal tube showed a 180 -volvulation of the sigmoid colon (Fig. 1). After a rectal enema with sodium chloride the patient passed a significant amount of stool and air, and his clinical condition improved instantly. A control X-ray clearly illustrated the devolvulation of the apparently elongated sigmoid (Fig. 2). The goal of this presentation is to demonstrate truly exclusive pictures delivering unequivocal direct evidence for
