Signs of the times Adrian Sugar Consultant and Senior Lecturer in Cleft and Maxillofacial Surgery, Morriston Hospital and Swansea University, Wales, UK.

It is with great pleasure that I welcome this new journal to the world community. It represents commitment, hard work, an understanding of the need to build all our clinical work on sound research foundations and above all it represents the bravery of those who have taken the initiative. The development of surgery of the head, neck, face and Mr. Adrian Sugar mouth in India is today moving at great pace. The country has many advantages. While the old industrialized countries are struggling with economic crises born of greed and a ‘get rich quick mentality’, India has seen huge economic development, wealth creation and scientific breakthroughs. It has mass scale so that, for example, in one unit it can treat almost as many children with clefts of the lip and palate in a year as are born in the same period of

time in the whole of the UK. Of course this has been greatly helped by the cleft charities, and the commitment of a few enlightened individuals; and it still has some way to go to offer the full range of lifetime care but what great steps we have seen over such a short period. And, India has such an educated professional class of clinicians and researchers who no longer depend on training solely in other countries and who also often have excellent facilities in which to work. However, what will India do with its advantages, which means effectively what will you do? Will it/you follow the same path as our countries into short termism and economic stagnation or will it/you use these gains to build a solid infrastructure for clinical development based on sound research for the benefit of all? It is a huge challenge and this journal offers a way to bring all our disciplines together on a multidisciplinary basis and to share that with colleagues the world over. I wish this endeavor great success and hope you will give it your support by publishing your work in it, by supporting its editors and by reading it.
