PURPOSE: To determine whether tumor vessel size or nnmhcr affects color Doppler flow detection in superficial melanoma metastsses. MATERIALS AND METHODS: Seven subcutaneous (n= 5) and superficial nodal (n= 2) melanoma metastascs in six patients were scanned with color Doppler US at 6 MHz. Color Doppler parameters were optimized for each lesion, and umscncc or absence of interns1 flow documented. Followinn &icsl removal, the metastascs underwent immunohistochemicsl staining for endothclial markers (Factor VIII and CD-34). For each lesion the number of tumor vessels (100X magnification) and diameter of the largest vessel (4COX magnification) were recorded. RESULTS: Four metsstases contained intcmsl color Doppler flow. and 3 did not. Histolonv showed the maximum vessel diamctcr to be significantly &ater (p&05) in lesions with flow, 73.6p +/- 19..5u, than in those without flow, 40.7u +/1 1.1 u. However. the mean number of tumor vessels in lesions with flow. 44.7 +/- 19.2, was not significantly different from those without flow, 63.5 +/- 50.6. CONCLUSION: Detectability of color Doppler flow in superficial melanoma metsstascs may be r&ted more to vessel diameter than to vessel number. However, further investigation is needed due to the small sample size.

Secondary hyperparathyreoidism is frequently encountered in permanently dialysed patients being result of phosphate retention and depression of intestinal calcium absorbtion. Material of our work consists of 21 patients with significant alterations of calcium & PTH serum levels (with PTH level over 400 mg/l) in whom ultrasound examination revealed l-4 cervical parathyroid adenomas varying in size from 6x4mm to 42x27mm. All pathological changes were injected with 96% apyrogen alcohol given in 2 parts during each session in total volume 1,5-2x exceeding the volume of hyperplastic glands. The procedure was usually repeated after l-3 months. Total amount of 34 tumors were cured with 70 injection attempts (54 complete injection, 12 partial injection, 4 failures). Clinical improvement & 3-4x decreased PTH level was observed in I5 patients, partial improvement in 4 while in 2 crises trere was no positive effect of alcoholisation. Repeated - 2 injections /I session ablation procedure enhances positive alcohol effect. Colour/power Doppler was extremely helpful in both initial study - demonstrating intratumoral & peripheral vessels in 29/34 adenomas as well as during repeated procedures (26/36). However its value was most pronounced in immediate control studies following ablation when flow was present in 8/70 tumors failures & partial injections. It was also a perfect indicator of active tumor in cases of repeated procedure of patients with multiple hyperplasia.

The purpose of this study was to determine sonographic features of diffuse goiter in order to differentiate thyroid dysfunctional disease from a simple endemic goiter. Fifly cases of diffuse goiter from a consecutive sample of 172 studies of thyroid ultrasonography were selected. Frequencies of sonographic parameters such as ecogenicity, ecotexture, contours and lobular thickness were determined. Besides, clinical laboratory tests, mainly serum levels of FT4, TSH and TPO ab were also measured in order to establish significant associations between groups according to the ecogeniclty and the dysfunctional parameters measured. The significance of differences between proportions was determined using Chi-square test. Twenty seven (54%) cases were Iso or Hiperecogenic (Group I) and 23 (46%) were hipoecogenic (Group II).
