FILMS Ultra Short-Acting Intravenous Barbiturate Amnesia for Urological Instrumentation Henry Bodner, Los Angeles, California. Dr. Bodner shows us how sodium methehexital can be used as an efficient, safe intravenous short-acting agent for urethral instrumentation. The actual action of the drug lasts only a minute and a half and, therefore, can only be used for procedures that would not take longer than that. The author states that it has been used in approximately 40,000 cystoscopies and other instrumentations without ill effect. Whereas the patient moves and cries out from pain, after rapid recovery the patient has no recollection of having had pain.

The film concerns the management of invasive penile carcinoma, and shows both the operative procedures for simple amputation of the distal portion of the penis wherein a guillotine type procedure is performed and the urethral cuff is spatulated. A second operation is also demonstrated wherein a radical penectomy is performed where the scrotum is split to get at the crura. The technique of groin dissection is also clearly demonstrated. The film is of excellent quality and well narrated.

The presentation, although somewhat repetitious at times, can be considered most interesting visually. The experimenter

has taken a dog kidney and has injected the vascular system with a silicone rubber material, Then, after tissue clearing, he has proceeded by microinjection of the glomeruli to demonstrate the tubules. The injection process, seen through the macroscope, is interesting and esthetically appealing. The main theme of the author is that the morphology of the basic units of renal function are far more complex than one would ordinarily assume from classic anatomic technics. Again the interest in this film centers around the superb macrophotography which is beautiful. This film is highly recommended for showings to people interested in the basic renal structure and physiology.

Suspension of the Female Urethra Using Tissue Adhesive Alpay Kelami, Berlin, Germany. This short, well-made film shows the operative technique for a modification of the Marshall-MarchettiKranz procedure, which consists of the usual dissection; however, at the time that the urethra and vagina are to be suspended, a tissue adhesive (n-butylcyanoacrylate) is used on the anterior surface of the urethra and vagina and using upward pressure by fingers in the vagina is made to adhere to the pubis. This is reinforced with several sutures, and the vagina is then packed. This appears to simplify the operation considerably and is worthy of further study. (A.Z.)
