Eagle Vision-Freeman™ Surgical Head and Hand Rest The Eagle Vision-Freeman™ Surgical Head and Hand Rest partially immobilizes the head during surgical procedures, while providing support for the surgeon's arms and hands. An insert is provided for children or smaller patients. The unit is made of medical grade Naugahyde®, custom stitched around foam rubber, and features velcro straps to anchor the headrest to any size operating table. Price on request Eagle Vision-Freeman™ Punctum Plug Kit The original Punctum Plug for use in treatment of keratitis sicca (dry eye) by reversible occlusion. The kit contains two (2) Punctum Plugs made of soft, flexible medical grade silicone rubber similar to that used to make soft silicone contact lenses. Plugs are non-toxic. Kit also includes one (1) Dilator /Inserter combination instrument and physician instructions. The procedure involves inserting the soft silicone plug into the puncta! opening of the patient's eyelid(s), thereby blocking this opening and eliminating tear loss by this route. The purpose is to permit more moisture to remain in the patient's eye(s). The plug can be removed with little or no side effects. The kit and contents are sterile and designed for single use. Now available in 3 sizes: standard size plug kit, medium size plug kit, and small size plug kit. Price on request

Temporary lntracanalicular Collagen Implant The implant procedure is a test which can be used in the diagnosis of a number of ocular disorders. including contact lens wearing problems. The collagen implants also may be used to temporarily enhance the effect of ocular medications. Occlusion of the lacrimal drainage system reduces the loss of topical medications into the outflow pathway, thus increasing the amount of medication available to ocular tissues. The implants are 2 mm in length and available in three diameters: 0.2 mm, 0.3 mm, and 0.4 mm. Six sterile implants are packaged in a lint-free foam holder, allowing implantation in all four canaliculi with two extra implants should they be dropped or mishandled. Each box contains twelve patient packs. The implants, which slow the excretion of tears into the nasal passages, are dissolved in 7-1 0 days. During this time, the collagen implants give the ophthalmologist and patient time to evaluate the benefit of increased tears on the surface of the eyes. Signs of improvement may suggest more permanent occlusion, such as offered by the total, yet reversible Eagle Vision-Freeman™ Punctum Plug. Price on request

Collagen Implant Forceps High-quality forceps for handling and inserting Temporary lntracanalicular Collagen Implants. The forceps are custom modified so that the tips are smooth and round, not sharp pointed. Tungsten carbide is impregnated into the inside surface of the tips to provide a secure grip on the implant. Price on request Cilia Forceps Swiss-manufactured epilation forceps with blunt tips and extended mating surfaces. Tungsten carbide has been impregnated into the mating surfaces to ensure a secure grip on the very finest cilia. Price on request EV™ Monocanalicular Stent For repair of single canalicular lacerations. This product combines an Eagle Vision-Freeman™ silicone Punctum Plug secured to medical grade silicone tubing for use as a stent in the repair of single canalicular lacerations. A standard size Punctum Plug is modified by having the end of the conical tip removed to create an open tunnel which converts the former plug to a small tube. This modified Punctum Plug then is attached by silicone glue to a 30-mm length of medical grade silicone tubing. The EV™ Monocanalicular Stent can be used to bridge the lacerated ends of the canaliculus, while providing an unobstructed outflow pathway for the tears during healing. Because the Punctum Plug remains fixed in the punctum, no sutures are needed to fixate the stent to the eyelid, in contrast to other products and techniques. Intubation into the nose and trauma to the uninjured canaliculus also are avoided. It has also been reported that the EV™ Monocanalicular Stent is useful in shortening the medial canthus where the patient suffers from ectropion. Other advantages include ease of insertion, good patient tolerance, and this treatment can be performed as an out-patient procedure in some cases. The EV™ Monocanalicular Stent is available in a sterile kit with one stent per kit. Price on request Medicool® Kit This portable refrigeration protection system keeps heat sensitive medication and eye drops below 70° F for over eight hours. Medicool® is small enough to fit in purse or briefcase and includes two freezable coolers and a sturdy carrying case with belt strap and pocket. Price on request Eagle Vision™ Silicone Lens Positioner With the advent of lens implants with no positioning holes, the surgeon at times encounters difficulty in positioning lenses. A lens positioner using soft silicone as the adhesion to move lenses has been developed. The product consists of a 27 -gauge irrigating cannula with approximately a 10-mm length of silicone tubing placed on the cannula. This cannula fits on any standard irrigating syringe and then is filled with either air or fluid (visco-elastic material). The Eagle Vision™ Silicone Lens Positioner then is placed

into the anterior /posterior chamber through the incision. Gentle pressure, using the silicone tip of the positioner, is placed close to the edge of the optic of the lens implant. Then pressure is applied in the direction of desired movement while exerting gentle posterior pressure. Advantages include the fact that the surgeon can irrigate or aspirate while positioning the lens. The soft tubing does not scratch, yet is reasonably adherent. Sold in a non-sterile package. Cleaning by standard technique and sterilization by autoclaving is recommended. Price on request EV™ Lid Cleanser EV™ Lid Cleanser is an effective cleanser for use whenever daily eyelid hygiene is indicated. It is excellent as an adjunct in conditions associated with acute and chronic blepharitis, and also ideal as an eye cosmetic remover, especially for contact lens wearers. Grieshaber & Co.

Sutherland Microforceps and Microscissors Sutherland forceps and scissors have been designed to be introduced through a 20 gauge incision. Nearly 30 instruments in all, these fine instruments offer the surgeon a variety of shapes and angles for anterior and posterior segment procedures. The tip of each instrument may be rotated in the eye without changing hand position. Widely used by posterior segment surgeons, rotatable microscissors and forceps are ideal for the management of vitreal membranes by segmenting or delamination. Ophthalmic Knives The Grieshaber Ultrasharp and Ultrathin lines offer the ophthalmologist a broad selection of sterile "disposable" microsurgical knives. Renowned for their exquisite and consistent sharpness, many ophthalmologists enjoy the economy of multiple use. The newly introduced Ultrathin group of knives offer lower costs and razor thin blade styles Ruby and Diamond knives provide a cutting edge possible only with a gem. A variety of blade styles are available and may be ordered with a retractable handle or as a replacement. Trocar Cannula System The Trocar Cannula System is designed to protect the sclerotomy site during vitreous surgery. Often the integrity of the sclerotomy is compromised by constant changing of the intraocular instruments. With the cannulas from the Trocar System placed in the incision, instruments, fiberoptics and infusion lines can be interchanged easily without stretching or enlarging the incision. The "twist-lock" connector of the cannulas allows for securing infusion and new fiberoptic lines easily, without the need for suturing.

Foreign Body Forcep (All-Purpose Diamond Coated) The Foreign Body Forcep was designed to provide the surgeon with one instrument capable of grasping most types of penetrating foreign bodies. The diamond chips lining the jaws make it easy to grip a foreign body of any composition or shape, providing ample gripping power to reduce the chances of a foreign body slipping off the tips, thereby causing additional trauma. Green-Line Bipolar Coagulator The Grieshaber Green-Line Bipolar Coagulator provides the latest development in solid state electronics. By producing a unique, pulsed square wave, enhanced coagulation is achieved utilizing less voltage and yielding even less destruction of surrounding tissue. Coagulant formation at the forcep tips is reduced as is TV and monitor interference. A complete line of ophthalmic forceps is available in addition to coaxial "Probes" for both anterior and posterior segment procedures. The new Grieshaber "ULTRALIGHT" cord reduces drag on the forcep or probe during delicate procedures. Manipulator The Grieshaber Manipulator is a unique three function instrument specifically designed for posterior segment surgery. Providing simultaneous illumination, coagulation and manipulation of tissue through a 1 mm probe, it eliminates the restrictions associated with a bimanual technique thereby creating a "third hand" for added versatility in the management of delicate procedures. The term Manipulator is derived from its ability to hold or manipulate tissue through its suction capability. The Manipulator is used with the Grieshaber Green-Line Bipolar Coagulator and Grieshaber Light Source, or adapts to most conventional systems. Grieshaber Air System The G.A.S. (Grieshaber Air System) is a self contained system that maintains intraocular pressure automatically without the need for syringes, bottles, air tanks, or the help of an assistant. It eliminates the danger of overfilling or collapse of the globe during fluid/gas exchange. The system is designed both for fluid/gas exchange in complex posterior segment surgery and for maintaining the anterior chamber. An audible alarm sounds when pressure varies beyond limits preset by the surgeon. A dual mode footswitch provides the surgeon with complete control. Accessory sterile tubing paks are available from Grieshaber to simplify hook-up for air delivery, or, with the "Global Control" pak, for fluid and air. MPC The Grieshaber MPC (Membrane Peeler Cutter) is an automated scissors which may be introduced through a 20 gauge incision. The shape and angle of the blades in the closed position allows for membrane peeling and delamination. The MPC reduces hand/

instrument movement and eliminates surgeon distraction that manual activation often requires. Cannulated Extrusion Needle The Cannulated Extrusion Needle is an instrument containing a 24 gauge silicone tube which can be extended for 18 mm beyond the tip of a 20 gauge needle. Although the instrument may be used in conjunction with an automated vacuum system via the standard bored tubing exiting from the end of the handpiece, passive egress of fluid is generally preferred and controlled with the venting hole found within the finger slide. OBF (Ocular Blood Flow Measuring) The OBF System, with its unique pressure sensor and menu-driven computer system, provides immediate graphic reports of pulsatile blood flow, vascular resistance, aqueous humor dynamics, perfusion pressure, and ophthalmic arterial pressure. This ability offers opportunities to modify pharmacological and surgical treatments and to measure their effectiveness. It also can help the physician avoid certain treatments when risk factors are brought to light. Prices on request

Karl Ilg Instruments, Inc.

Karl and Paul llg have over 40 years of experience producing surgical eye instruments. They are ophthalmic surgical designers and are prepared to give their personal attention to any concept you may have concerning the design of new instruments or modifications of existing instruments. Some of our newest designs for this year are: Shepard Irrigating Capsule Polisher has a 2-mm I.D. cup with 23 G irrigation directly out of the distal end. The polisher has a gradual 40° curve upward. Back side of polisher is thinned out to reduce height and highly polished to minimize damage to the endothelium. Simcoe (Modified Shepard) Large Aperture Cannula 20 G TW aspiration with 0.5- or 0.7-mm hole on top side thru tubing attachment to a syringe. 20 G TW irrigation thru luer-lok hub, which can be attached to hanging I.V. bottle or thru a handle. This large hole is designed for aspiration of the fractured nucleus or soft immature nucleus. Simcoe (Modified Shepard) Large Aperture Cannula is also available with 0.5- or 0.7-mm hole on the side, instead of the top side.

Lieberman Non-ridged Lens Implant Forceps This is used to hold securely 6- or 7-mm biconvex and planoconvex no-hole lenses. The instrument has a 5" long round handle to hold comfortably in the surgeons hand. The bottom blade has a rounded offset tip to hold lenses firmly so there is no side-to-side motion. The upper blade is concaved so it conforms to the lens. The instrument has a stop in the back to prevent the forceps from being bent out of shape. It is available with round handle, flat handle, or crossaction. The Kendrick-Mims Nucleus Manipulator This is used in extracapsular surgery to loosen the nucleus before expression or loop removal of the nucleus of the crystalline cataractous lens. It can be used to deliver the nuclei into the anterior chamber. It is also an excellent iris repositor, and useful for other intraocular manipulations. Kietzman Lens Haptic Loop Designed for more assured in-the-bag placement of C-loop intraocular lenses. The loop is threaded over the superior haptic after the inferior haptic is in place in the bag inferiorly. The loop is held in place with the left hand where the superior haptic is to enter the bag while the lens is dialed into a horizontally oriented position with a modified Lester hook held in the right hand. In addition, we offer these popular sellers: Lieberman-Tennant Tying Forceps with smooth fine jaws, burnished tips (straight or curved) 4W' or 5" long round handle Lieberman Capsule Polisher, 1-mm cup, with iris retractor and irrigation through center of cup. The Karickhoff Dobule 1/A Cannula has a gradual 45° curve upward, 21-G irrigation out front and 23-G aspiration on top, near the tip, with a 0.3-mm hole. Putterman Clamps for the treatment of ptosis by the MUller Muscle conjunctival resection operation of Putterman and Urist. Putterman Levator Resection Clamps designed to facilitate the resection of the levator muscle, by the external or internal-external approaches. The Lieberman Phaco Crusher with Offset Tip and the Lieberman Ph aco Crusher with Y-Shaped Tip. These two modified spatulas/manipulators, improve the effectiveness of your phaco technique by dividing the lens nucleus with small segments and then crushes each segment at the U/S tip. The Clamp for Cicatricial Entropion Surgery. This clamp has been designed to simplify the performance of the Wies and Ballen transmarginal rotation entropion procedures. The clamp maintains hemostasis throughout the initial portions of the procedure. The clamp is shaped for use in a partially obliterated fornix. The Judson Smith Manipulator This is a modified Graether collar button, with a positioning stud. It is used to dial peripheral "tab" type intraocular lenses with peripheral

Additional literature and pricing available on request. New Crawford Intubation Set with Suture The original Crawford Intubation Set, designed by J. S. Crawford, MD, was developed to facilitate the insertion of lacrimal intubation tubing while reducing trauma and tearing of the nasal floor. This was achieved through use of two flexible, stainless steel wires (.016" diameter), silastic tubing (.025" diameter), and a specially designed hook. Now, a newly developed Crawford Intubation Set is available which incorporates a suture running through the center of the silastic tubing, allowing a small surgical knot to hold the tubing in place. This feature is beneficial when removing the tubing from either the canaliculi or from the nasal passage since the suture is discreet and can be removed in the office without anesthesia. Additional literature and pricing available on request. Keratoscopes Two models of surgical keratoscopes provide a costeffective alternative to expensive surgical kertometers. Preoperatively, the keratoscopes may be utilized to confirm previously recorded K readings. Intraoperatively, the keratoscopes facilitate the rapid measurement of astigmatism, affording accurate suture adjustment. In addition, the keratoscopes yield an immediate qualitative estimate of the effects of astigmatic keratotomy. Available in a sterile, disposable model, packed six (6) to a box, (Cat. No. 28-4415), and a fully autoclavable, stainless steel model (Cat. No. 28-4425). Additional literature and pricing available on request.

Rx Honing Machine Corporation

Honing/Sharpening Machine for Surgical Instruments Rx Honing Machine Corp. introduces its new Model 2 Rx System II instrument sharpener. The Model 2 machine has been specifically designed for high performance. It utilizes proven technology developed from Rx Honing's 21 years of hands-on research and development. Honing processes developed by the company allow for easy maintenance of instrument cutting edges without danger of harming temper or hardness. The alternative to Rx System II is the grinding machine which is likely to destroy temper. The Rx System II honing processes enable the sharpening of the complete line of surgical instruments, even the finest micro surgical instruments. The machine includes multiple drives for its honing stones: reciprocating, oscillating, disk, and now a new flex shaft. The flex shaft drive simplifies the sharpening of cupped edges such as bone rongeurs, basket forceps, bone curettes, and kerrison punches. The Rx System II is simple to use. Instructions for the Sharpening Set are contained in the operators manual, and in an easy-to-follow video casette program. An optional one-day training course is also available through Rx Honing at a nominal fee, Rx customers all over the world have achieved substantial reduction in instrument replacement and maintenance costs, and have gained optimum instrument quality.

Sutures Alcon Surgical Products Division

The Alcon Closure System, ACS®, is a line of needles and sutures produced especially for the ophthalmic surgeon. The sutures presented are either nylon, silk or dacron. Included in the line are several unique specialty needles. A description follows. Mcintyre Fishhook Needle-Designed by David Mcintyre, M.D., and the first bi-curve ophthalmic needle. The needle produces a short deep bite helpful in controlling post surgical astigmatism. Tennant goo Needle-Designed and patent applied for by Jerald Tennant, M.D., this needle incorporates a goo bend 1 mm from the tip. The resulting suture track is square thus reducing vector forces and also helping to reduce postoperative astigmatism. CU-8 is a bi-curve needle with a less radical first curve than the Fishhook needle and also producing a deep narrow bite. The needle was suggested by R. G. Martin, M.D., and has become a very popular needle for corneal closure. It is easy to use and does not require a change in surgical technique. CUM-15 is a miniature CU-15 needle whose design was suggested by R. Torchia, M.D., and J. Hunkeler, M.D. The needle has a 3 mm chord and produces a very short bite. It has a 0.004" thick wire which produces the smallest of needle tracks. "A" Series is a new series of ophthalmic needles which has been introduced to ophthalmology. This series, for corneal/scleral closure, employs a new tip design and a trapezoid body for increased sharpness and added strength. "R" series of retina needles are flat, side cutting needles in goo and 180° curves for use in a buckling procedure and the encircling band procedure. The needle cuts between

lamella and not down toward the retina. An ideal series for the retina/vitreous surgeon. PAIR-PAK™ is a unique product employing two entirely different needles on one suture. The product is used as a bridle suture and as a safety;temporary suture, giving the surgeon convenience and economy. All Alcon needles are produced to exacting specifications and are known for their lasting sharpness pass after pass. Needle selection is a personal preference based on technique and skill. Therefore, a wide variety of products is available. Prices on request
