Sutures Alcon Surgical
The Alcon Closure System, ACS", is a line of needles and sutures produced especially for the ophthalmic surgeon. The sutures presented are either nylon, silk or dacron. Included in the line are several unique specialty needles. A description follows. Mcintyre Fishhook Needle-Designed and patent applied for by David MCintyre, M.D., and the first bi-curve ophthalmic needle. The needle produces a short deep bite helpful in controlling post surgical astigmatism. Tennant 90° Needle-Designed and patient applied for by Jerald Tennant, M.D., this needle incorporates a 90° bend 1 mm from the tip. The resulting suture track is square thus reducing vector forces and also helping to reduce postoperative astigmatism. CU-8 is a bi-curve needle with a less radical first curve than the Fishhook needle and also producing a deep
narrow bite. The needle was suggested by R. G. Martin, M.D., and has become a very popular needle for corneal closure. It is easy to use and does not require a change in surgical technique. CUM-15 is a miniature CU-15 needle whose design was suggested by R. Torchia, M.D., and J. Hunkeler, M.D. The needle has a 3 mm chord and produces a very short bite. It has a 0.004" thick wire which produces the smallest of needle tracks. "R" series of retina needles are flat, side cutting needles in 90° and 180° curves for use in a buckling procedure and the enCircling band procedure. The needle cuts between lamella and not down toward the retina. An ideal series for the retina/vitreous surgeon. All Alcon needles are produced to exacting specifications and are known for their lasting sharpness pass after pass. Needle selection is a personal preference based on technique and skill. Absorbable Sutures Plain Catgut Plain catgut is a suture material prepared from submucosal or mucosal layers of sheep or beef intestines which consist primarily of a collagen fibrous protein and is absorbed by the body. This material is chemically purified to minimize tissue reaction. This is currently available in sizes 4/0 through 6/0 Chromic Catgut Chromic catgut is the same as plain catgut except it is additionally treated by chromium salts to delay absorption time. This is available in sizes 4/0 through 7/0 Plain Collagen Plain collagen is prepared from bovine deep flexOr tendons. The tendon is purified and converted to a uniform suspension of collagen fibril. This fibrillar suspension is then extruded into suture strands and chemically treated to accurately control absorption rates. This is available in sizes 4/0 through 7/0 Chromic Collagen Chromic collagen is prepared in the same way as plain collagen except that chromium salts are added during the chemical treatment to further delay absorption. This is available in sizes 4/0 through 7/0 SynthetiC Absorbable Sutures: VICRYL'· (Polyglactin 910) Suture and Coated VICRYL" (Polyglactin 910) Suture, PDS* (Polydioxanone) Suture VICRYL suture and Coated VICRYL suture materials are a copolymer of lactide and glycolide. This

material offers high tensile strength during critical postoperative healing with minimal tissue reaction followed by predictable absorption. It is interesting to note that the absorption of VICRYL suture and Coated VICRYL suture may be considered to occur in two distinct phases. After the suture is implanted, the tensile strength diminishes during the early postoperative period. After most of the strength is gone, the remaining suture mass which is still present begins to decrease, which may be termed as the second phase of the absorption. The mass loss phase then proceeds until the entire suture has been absorbed. Coated VICRYL sutures are made with a VICRYL strand coated with a polyglactin 370 coating (30 percent glycolide, 70 percent lactide and a combination of calcium sterates). The advantage of the coating is smoother passage through tissue and greater knot security. These synthetiC absorbable materials are available in sizes 4/0 through 10/0 in uncoated sutures and 4/0 through 8/0 in coated sutures PDS· (Polydioxanone) Suture This material has a stronger in vivo tensile strength than other available synthetiC absorbables. It is a monofilament which allows for smooth passage through ocular tissue. It is available in size 9/0 Non-Absorbable Sutures ETHILON'· Black Monofilament Nylon Suture This black monofilament nylon has a high tensile strength and minimal tissue reaction. Nylon has been reported to lose its tensile strength postoperatively at a rate of approximately 10 to 15 percent per year. This is currently available in sizes 8/0 through 11/0

PROLENE'" Polypropylene Suture PROLENE sutures are monofilament with a high tensile strength and minimal tissue reaction. These sutures are not degraded or weakened by tissue enzymes. PROLENE suture is available in sizes 5/0, 9/0 and 10/0 Black Braided Silk Sutures ETHICON'· silk is braided under controlled conditions to maximize strength and assure resistance to breaking during knot tying. Gums and other impurities are removed, resulting in a suture that remains tightly braided with virtually no loose filaments and minimal tendency to broom. Available in sizes 4/0 through 9/0 Virgin Silk Virgin silk sutures are twisted from the individual silk filament still embedded in the natural sericin coating providing a smooth, uniform suture in very fine sizes. They are offered in black and white for selection of optimal contrasts with tissues. Virgin silk sutures are available in sizes 8/0 and 9/0 MERSILENE'" Polyester Fiber Sutures This dacron suture material exhibits minimal tissue reaction and is braided by a special method for tightness, uniformity and a smooth surface that minimizes trauma. This is available in sizes 4/0 through 6/0 New Monofilament MERSILENE* polyester fiber suture is 50% stronger than nylon. It will not hydrolyze over time. It is available in size 10/0. This product is less elastic than other monofilaments, which makes it ideal for Keratoplasty as well as Cataract Extraction. Needles ETHICON ophthalmic needles are manufactured from fine-grade stainless steel. Five different types of needle edges and pOints are avaiable from Ethicon, Inc. They are: Taperpoint Needles Desirable when minimal tissue trauma is indicated. Applications for taperpoint needles are intraocular lens fixation and superior rectus traction sutures TAPERCUT'" Cutting Needles This needle's tip is honed to a cutting surface while the balance of the body is tapered. This needle is indicated for use in initially tough to penetrate tissues and yet minimal trauma by the balance of the needle's passage is experienced. The surgical uses for such a configurated needle are retinal detachments and intraocular lens refixations Reverse Cutting Needles These needles are of a triangular figuration with the cutting edge on the bottom of the needle. These particular needles are used where tough, fiberous tissues will be encountered Blunt POint Needles Manufactured with a tapered body which blends into a blunt, non-tissue penetrating pOint. Surgical uses for such a needle are intraocular lens fixation and intubation of lacrimal ducts

Sutures Alcon Surgical
The Alcon Closure System, ACS", is a line of needles and sutures produced especially for the ophthalmic surgeon. The sutures presented are either nylon, silk or dacron. Included in the line are several unique specialty needles. A description follows. Mcintyre Fishhook Needle-Designed and patent applied for by David MCintyre, M.D., and the first bi-curve ophthalmic needle. The needle produces a short deep bite helpful in controlling post surgical astigmatism. Tennant 90° Needle-Designed and patient applied for by Jerald Tennant, M.D., this needle incorporates a 90° bend 1 mm from the tip. The resulting suture track is square thus reducing vector forces and also helping to reduce postoperative astigmatism. CU-8 is a bi-curve needle with a less radical first curve than the Fishhook needle and also producing a deep
narrow bite. The needle was suggested by R. G. Martin, M.D., and has become a very popular needle for corneal closure. It is easy to use and does not require a change in surgical technique. CUM-15 is a miniature CU-15 needle whose design was suggested by R. Torchia, M.D., and J. Hunkeler, M.D. The needle has a 3 mm chord and produces a very short bite. It has a 0.004" thick wire which produces the smallest of needle tracks. "R" series of retina needles are flat, side cutting needles in 90° and 180° curves for use in a buckling procedure and the enCircling band procedure. The needle cuts between lamella and not down toward the retina. An ideal series for the retina/vitreous surgeon. All Alcon needles are produced to exacting specifications and are known for their lasting sharpness pass after pass. Needle selection is a personal preference based on technique and skill. Absorbable Sutures Plain Catgut Plain catgut is a suture material prepared from submucosal or mucosal layers of sheep or beef intestines which consist primarily of a collagen fibrous protein and is absorbed by the body. This material is chemically purified to minimize tissue reaction. This is currently available in sizes 4/0 through 6/0 Chromic Catgut Chromic catgut is the same as plain catgut except it is additionally treated by chromium salts to delay absorption time. This is available in sizes 4/0 through 7/0 Plain Collagen Plain collagen is prepared from bovine deep flexOr tendons. The tendon is purified and converted to a uniform suspension of collagen fibril. This fibrillar suspension is then extruded into suture strands and chemically treated to accurately control absorption rates. This is available in sizes 4/0 through 7/0 Chromic Collagen Chromic collagen is prepared in the same way as plain collagen except that chromium salts are added during the chemical treatment to further delay absorption. This is available in sizes 4/0 through 7/0 SynthetiC Absorbable Sutures: VICRYL'· (Polyglactin 910) Suture and Coated VICRYL" (Polyglactin 910) Suture, PDS* (Polydioxanone) Suture VICRYL suture and Coated VICRYL suture materials are a copolymer of lactide and glycolide. This

material offers high tensile strength during critical postoperative healing with minimal tissue reaction followed by predictable absorption. It is interesting to note that the absorption of VICRYL suture and Coated VICRYL suture may be considered to occur in two distinct phases. After the suture is implanted, the tensile strength diminishes during the early postoperative period. After most of the strength is gone, the remaining suture mass which is still present begins to decrease, which may be termed as the second phase of the absorption. The mass loss phase then proceeds until the entire suture has been absorbed. Coated VICRYL sutures are made with a VICRYL strand coated with a polyglactin 370 coating (30 percent glycolide, 70 percent lactide and a combination of calcium sterates). The advantage of the coating is smoother passage through tissue and greater knot security. These synthetiC absorbable materials are available in sizes 4/0 through 10/0 in uncoated sutures and 4/0 through 8/0 in coated sutures PDS· (Polydioxanone) Suture This material has a stronger in vivo tensile strength than other available synthetiC absorbables. It is a monofilament which allows for smooth passage through ocular tissue. It is available in size 9/0 Non-Absorbable Sutures ETHILON'· Black Monofilament Nylon Suture This black monofilament nylon has a high tensile strength and minimal tissue reaction. Nylon has been reported to lose its tensile strength postoperatively at a rate of approximately 10 to 15 percent per year. This is currently available in sizes 8/0 through 11/0

PROLENE'" Polypropylene Suture PROLENE sutures are monofilament with a high tensile strength and minimal tissue reaction. These sutures are not degraded or weakened by tissue enzymes. PROLENE suture is available in sizes 5/0, 9/0 and 10/0 Black Braided Silk Sutures ETHICON'· silk is braided under controlled conditions to maximize strength and assure resistance to breaking during knot tying. Gums and other impurities are removed, resulting in a suture that remains tightly braided with virtually no loose filaments and minimal tendency to broom. Available in sizes 4/0 through 9/0 Virgin Silk Virgin silk sutures are twisted from the individual silk filament still embedded in the natural sericin coating providing a smooth, uniform suture in very fine sizes. They are offered in black and white for selection of optimal contrasts with tissues. Virgin silk sutures are available in sizes 8/0 and 9/0 MERSILENE'" Polyester Fiber Sutures This dacron suture material exhibits minimal tissue reaction and is braided by a special method for tightness, uniformity and a smooth surface that minimizes trauma. This is available in sizes 4/0 through 6/0 New Monofilament MERSILENE* polyester fiber suture is 50% stronger than nylon. It will not hydrolyze over time. It is available in size 10/0. This product is less elastic than other monofilaments, which makes it ideal for Keratoplasty as well as Cataract Extraction. Needles ETHICON ophthalmic needles are manufactured from fine-grade stainless steel. Five different types of needle edges and pOints are avaiable from Ethicon, Inc. They are: Taperpoint Needles Desirable when minimal tissue trauma is indicated. Applications for taperpoint needles are intraocular lens fixation and superior rectus traction sutures TAPERCUT'" Cutting Needles This needle's tip is honed to a cutting surface while the balance of the body is tapered. This needle is indicated for use in initially tough to penetrate tissues and yet minimal trauma by the balance of the needle's passage is experienced. The surgical uses for such a configurated needle are retinal detachments and intraocular lens refixations Reverse Cutting Needles These needles are of a triangular figuration with the cutting edge on the bottom of the needle. These particular needles are used where tough, fiberous tissues will be encountered Blunt POint Needles Manufactured with a tapered body which blends into a blunt, non-tissue penetrating pOint. MICRO-POINT'" Reverse Cutting Needles Like cutting needles, they are triangular in configuration with the cutting edge on the bottom of the needle. These needles were the first generation of needles designed for ophthalmology. Their main surgical uses today are for traction sutures, as well as oculoplastic closure. Supramid Extra® Sutures for ophthalmic surgery are available with premium reverse-cutting, taper pOint, and spatulated as well as micro needles. Supramid Extra® suture is available in all sizes from 10-0 and larger. These nylon sutures are monofilament from size 10-0 to size 5-0. Sizes 4-0 and larger have a unique cable-type construction which eliminates "memory" and gives unsurpassed knot holding ability with a minimum of knots.
