To establish and validate laboratory criteria for the differential diagnosis of primary (essential) thrombocythemia, (ET) vs. secondary thrombocytosis (ST), we prospectively evaluated 9 cases of ET and 10 of ST. The diagnosis was confirmed at one year follow-up of patients showing the permanence or the regression of thrombocytosis, respectively. Bleeding time, platelet adhesiveness, circulating platelet aggregates, platelet aggregation in response to ADP, adrenaline, collagen, arachidonic acid and calcium ionophore A23187, 8-thromboglobulin, platelet factor 4 and generation of thromboxane B2 after spontaneous clotting were performed in all cases.
