A 25-year-old woman suffered a severe whiplash injury after a car accident. (A) Computed tomography scan indicated a long dissection of the left internal carotid artery (LICA), while on the right side there was active bleeding around the common carotid artery (CCA) with segmental occlusion of the right internal carotid artery (RICA). (B) At open exploration a near-total transection of the carotid bifurcation with local thrombosis of the RICA were demonstrated, whereupon a vein interposition was performed to re-establish RICA flow, with ligation of the right external carotid artery (RECA). Dissection of the LICA was managed conservatively. Recovery was uneventful with no neurological complications.
