Based on the model originally published in this journal in the article "Epidemiology of Medication-Related Adverse Events in Nursing Homes" by Handler, Wright, Ruby, and Hanlon ( A m J Geriatr Pharmacother. 2006:4:264-272), each bibliography will be divided into 2 major areas: medicationrelated adverse patient events and medication errors. The medication-related adverse patient events area will contain 3 subdivisions: adverse drug reactions; adverse drug-withdrawal events; and therapeutic failure. The medication-errors area will have 6 subdivisions: prescribing; order communication; dispensing; adherence; administration; and monitoring. If no relevant publications are identified within a particular section during the relevant period, that section will be omitted.
