Clinique Saint Augustin, Dept. of Urology, Bordeaux, France INTRODUCTION & OBJECTIVES: Continent catheterizable appendicovesicostomy is indicated in patients who suffer from bladder emptying disorders, such as neurogenic diseases, disorders of the lower urinary tract, or idiopathic causes, where catheterization per uretram is unfeasible. When native bladder compliance is significantly reduced, concomitant augmentation ileocystoplasty is indicated. Nowadays, very few reports of laparoscopic bladder augmentation and Mitrofanoff appendicovesicostomy have been published, at our knowledge only in the pediatric setting. We describe our technique in adult patients. MATERIAL & METHODS: We present the case of a 28-yrs female patient, suffering from post-traumatic C5 tetraplegia, with a bladder capacity of 200 cc and vesico-sphincter dyssynergia. RESULTS: Surgery went well and the operative time was 248 minutes. The postoperative course was uneventful. After a follow-up of 2 years, the patient is perfectly continent and able to easily perform auto-catheterization. CONCLUSIONS: A laparoscopic approach can be recommended for the realization of this kind of technically demanding procedures, if a standardized and reproducible technique is followed.
