43 Therapeutic Challenge
45
Onychomycosis and tinea pedis are exceedingly common conditions experienced by patients. In addition
46
to itch and cosmetic concerns, they increase the risk of cellulitis and diabetic foot ulcers1. While nightly
47
50/50 vinegar soaks can increase efficacy of topical antifungals due to fungicidal effects of acidic pH2,
48
the inconvenience of tub soaks limits compliance.

68
Each evening, patients go to a bathroom or other place with a vinegar-resistant floor, don old cotton
69
socks, pour a few spoons of plain or apple-cider vinegar and a few spoons of water over the toes, and sit
70
and read for 10-15 minutes. They then doff the socks, apply topical antifungals, and go to bed. In our
71
experience, compliance, efficacy, and satisfaction are high with most patients achieving complete
72
clinical responses within a year.
