The technique involves insertion of a lubricated gum elastic bougie through the GlideScopeÒ. Firstly, the bougie has a distal tip angulated at 30 anteriorly, therefore, it is easier to place the bougie in the trachea. Secondly, the bougie tip can be easily manipulated by gently rotating movements. Third, the bougie is almost atraumatic. Fourth, the technique can be routinely used to increase the successful intubation through the GlideScopeÒ.
, To the Editor: I read with interest the article by Walls et al. (1). The authors described a maneuver to overcome difficult placement of a tracheal tube through a GlideScopeÒ video laryngoscope (Verathon, Bothell, WA). The authors described insertion of a reshaped tube using a metallic stylet. The tube is inserted into the glottis and then rotated by 180 . Although one may overcome difficult angulations using this maneuver, an injury to the airway cannot be ruled out due to the 180 rotation of the tracheal tube. This is important not only during intubation but also during removal of the metallic stylet from the tracheal tube. I suggest a simpler, more effective, and safer alternative method to overcome the intubation difficulty in such cases.
