(normal is 8.0 to 10.5 mg/dL), a significantly elevated alkaline phosphate level at 1218 IU/L (normal, 150 to 400 IU/L), and a total phosphorus level at the high end of normal at 5.9 mg/dL (3.8-6.2 mg/dL). Her hypocalcemia was evaluated by testing her ionized calcium level, which was 3.3 mg/dL (normal, 4.0 to 4.7 mg/dL). Because of

the low ionized calcium level, radiographs of her long bones were obtained to search for radiographic evidence of rickets. The long bone radiograph revealed widening, cupping, and fraying of her metaphyses consistent with the diagnosis of rickets. A chest radiograph revealed mild cardiomegaly, and an echocardiogram

showed a structurally normal heart with a small plural effusion. Ann underwent upper gastrointestinal (UGI) radiography because of her history of spitting up after being fed and poor oral intake. Results of the UGI series were negative, and an esophageal pH probe revealed minimal gastroesophageal reflux.
