findings highlight the importance of inflammatory pathways in the pathophysiology of calcium oxalate nephrocalcinosis induced renal damage. Inflammasome induced cytokine release may also have a role in this process. Dean G. Assimos, MD

Re: Hypercalcemia, Hypercalciuria, and Kidney Stones in Long-Term Studies of Vitamin D Supplementation: A Systematic Review and Meta-Analysis Z. Malihi, Z. Wu, A. W. Stewart, C. M. Lawes and R. Scragg School of Population Health, University of Auckland, Auckland, New Zealand Am J Clin Nutr 2016; 104: 1039e1051. doi: 10.3945/ajcn.116.134981

Abstract available at http://www.ncbi.nlm.nih.gov/pubmed/27604776 Editorial Comment: This meta-analysis demonstrated that vitamin D supplementation was not associated with a risk of kidney stones. However, there was an increased risk of hypercalciuria and hypercalcemia. Previously reported metabolic studies have not shown that vitamin D supplementation in those with deficient vitamin D levels augments urinary calcium excretion. Dean G. Assimos, MD

Abstract available at http://www.ncbi.nlm.nih.gov/pubmed/27671898 Editorial Comment: These investigators assessed the clinical usefulness of a disposable digital flexible ureteroscope. Overall the device performed well, although there was device failure in 5% of cases. This instrument has time limited use. A cost analysis should be done at institutions contemplating using this device to determine whether there is an economic advantage. Perhaps this instrument can be selectively used in cases where a long operative time is anticipated as it may limit damage to “nondisposable scopes” and reduce high cost repairs. Dean G. Assimos, MD
