Renal pathology with clinical and functional correlations

Renal Pathology With Clinical and Functional Correlations. C. C. Tisher and B. M. Brenner. Philadelphia, PA, Lippincott, 1994, 1,694 pages, $250. This second edition of this textbook of renal pathology extends and updates the highly acclaimed first edition of this work. The editors state, "The rapid pace of progress in the scientific and clinical aspects of renal pathology since 1989, especially in the terms of cell biology, immunology, and molecular genetics, warrants the extensive updating and expansion that characterize this new edition." Thus, they undertook a complete update and revision of the book. As before, the chapters have been written by internationally renowned experts who have provided foundational material as well as the most up-to-date information available. This two-volume book is divided into seven sections. Part one, entitled "General Considerations," includes a detailed section on the gross anatomy of the kidney as well as renal development and the molecular aspects of organogenesis. A new chapter has been added covering the serological diagnosis of kidney disease. Both the clinical indications for renal biopsy and the evaluation of renal biopsy specimens are discussed in separate chapters. Finally, factors involved in the progression of renal disease are covered. The second section is entitled "Glomerular Diseases." After introductory chapters on the structure and function of the glomerulus and immunopathogenic mechanisms of glomerular injury, detailed chapters cover the major primary and secondary glomerular diseases. The third section, entitled "Tubular and Interstitial Diseases," includes 11 chapters. The introductory chapter covers the structure and function of the renal tubule and interstitium and the following chapters cover everything from acute tubular necrosis to tuberculosis. The fourth section covers the vascular diseases and begins with a review of the normal structure and function of the renal vasculature followed by a discussion of mechanisms of vascular injury. Five chapters follow covering specific vascular diseases that particularly involve the kidney, including scle-
roderma, the arteritides, thrombotic microangiopathy, renal vein thrombosis, and renal vascular disease. Section five is entitled "Renal Involvement in Heredofamilial, Metabolic and Hematologic Diseases." The chapters in this section are rather loosely linked, covering a wide variety of renal diseases. There are detailed discussions of heredity renal diseases such as Alport's syndrome, lipodystrophy, renal cystic disease, and congenital malformations. Also included is an excellent chapter on nephrotic syndrome in the first year of life. This section concludes with chapters on diabetic nephropathy, uric acid nephropathy, and paraprotein and lymphoma mediated renal diseases as well as sickle cell anemia and polycythemia-related renal injury. Part six includes chapters on renal neoplasms in pediatric and adult patients and part seven covers the renal allograft in detail, including a chapter devoted to cyclosporine nephrotoxicity. An excellent appendix discusses the handling and processing of renal biopsies and nephrectomy specimens. Each of the chapters almost could serve as an indMdual monograph. The photomicrographs {several pages of excellent color plates have been added in this edition) and supporting charts and tabular information are excellent. The up-to-date and extensive reference sections are extremely helpful. But perhaps the most beneficial aspect of this work is the extensive discussion of not only the "typical" manifestations of a particular disease entity but also the rare and unusual wariants; indeed, it is these exceptions to the rule that cause the most diagnostic difficulty. As with any multiauthored textbook there is some stylistic variation from chapter to chapter. There are also many areas where individuals might quibble with what was included or left out. However, for the most part these are really minor distractions. The detail provided in the text and the invaluable updated reference material ensures daily active use in a renal biopsy practice. This book also serves as an excellent reference on renal pathology for the surgical pathologist and should be required for the reference libraries in pathology departments training anatomic pathology residents.--PATPaCK D.
