INSTRUCTIONS Objectives: After reading the designated articles published in Fertility and Sterility®, participants of the Fertility and Sterility® Continuing Medical Education (CME) Program should be able to: 1. Demonstrate an increased knowledge of reproductive medicine. 2. Evaluate the clinical data and apply it to the care of their patients. Target Audience: This activity is designed for all physicians with a fundamental interest and knowledge in reproductive medicine, residents, researchers, clinicians, and educators alike. Accreditation: The American Society for Reproductive Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. The American Society for Reproductive Medicine designates this educational activity for a maximum of 15 AMA PRA Category 1 Credit(s)™. Physicians should only claim

credit commensurate with the extent of their participation in the activity. Instructions: In each issue, a separate CME section will be published in which the reader will find learning objectives and multiple choice questions for each of the designated articles. Program participants should answer the questions on the answer sheet provided in the June and December issues and return the answer sheet with the registration fee. The program is provided at a cost of $45.00 for each 6-month period of participation. The participant should retain a copy of the examination. A 70% passing score must be achieved. Successful participants will receive a letter of satisfactory completion, the correct answers, and a course evaluation form.
