to confidently go ahead with lap. Cholecystectomy even in acute setting without any hesitation. Regarding suggestion about revising Tokyo guidelines in order to assess the risk in relation to the patient conditions and not only to the AC severity, this is really a valid observation. In fact this point is raised in many other studies as well.

1.2. Comments to the author Although good number of studies do exist about lap. Cholecystectomy usually regarding early vs delayed but this meta-analysis has chosen a different subject, lap. vs open in acute cholecystitis. The conventional opinion in regarding this hot issue goes in favour of open, citing different reasons as highlighted by authors. This meta-analysis disproves such beliefs and this one can help surgeons
